Who's had an oil filter pop off or other failure?

+1 vote for aftermarket filter working loose.
Mahle filter (Porsche suppliers, apparently) supplied and fitted by Chris Franklin at CoG, stayed in place for a fortnight or so including driving with high engine speed, unthreaded itself on a trackday. Didn't lose too much oil though and oil light never illuminated. Seal was intact and filter screwed back on and stayed put for a day or two until I could get an OEM one fitted.
Chris Franklin kept me posted regarding the report he'd made to Mahle following both mine and Funster's filter incident - he informed me that Mahle had actually altered the design as a result...
In response to the thread that prompted this poll; I think the advice; "Honda one? Nah, just get one from your local motorfactors." sounds a bit cavalier.
Why bother using any other than OEM if it could possibly void your warranty? The Mahle incident(s) prove that value/quality/prestige aren't really relevant factors where the suitability of a filter is concerned.

Turns out that Honda have made revisions to their OEM Filter too.
It also seems that it is manufactured by FRAM, but was manufactured by Filtech (at least in US!).

In my experience the most common cause of coming loose is not lubing the seal before fitting, this causes the new rubber to grab when tightening giving the impression its tight. Sadly often after a bit of vibration, some extra heat (trackday for example) and some pressurised oil, the unlubed seal looses grip and allows the filter to come loose.
I have never seen a properly fitted filter come loose irrespective of make.

Didn't Honda revise theirs for commonality? I think one fits all?

If I am wrong and that there as many problems with OEM as non OEM, I will hppily admit i'm wrong. Not so sure you will accept it?

And the one chris franklin fitted, i think there was a thread about that one, if i remember it was fitted to a newish car.
