its just the mechanism inside the caliper, brakes are OK and probably the cables move ok(check this first) as the pads wear the piston sits further out of the caliper to keep the face of the pad in contact with the disc, if the auto adjust does not work the movement of the handbrake fails to make contact with the piston so the pads dont move. The fact that the normal brakes are ok means the self adjustment for the pads is working its just the "ratchet" mechanism that clicks up a notch if the hand brake movement exceeds a certain distance, this is probably worn or gummed up with crap.
