Actually, insurance companies not only can randomly deny coverage, they often do. The sad thing is that 10% of people who are wrongfully denied insurance coverage for a loss never fight the first claim denial. I'm not suggesting that this is legal or written down in procedural manuals somewhere, but you'd be naive to believe that it doesn't happen regularly.
Never forget that the insurance companies are in business to make money. They don't make money by paying out on claims. Every time they deny a claim that they should have paid and aren't challenged, their bottom line grows a little bit.

I had state Farm when the bike was stolen, and it was no where near as difficult. My claim was settled in 5 days.
She steers me how to save $, what extra stuff to have on policies, etc, and never bullshits me or tries to sell me shit I don't need. Just recently they changed flood damage stuff for all of Texas and I got a phone call the same day.

Been with them for almost a decade. And I can tell you that having a good relationship with your local insurance agent pays big dividends, and I mean big! My lady calls me anytime they (State Farm) come out with changes to homeowners/auto, or addendums.

I moved my auto insurance away from them four years ago, but kept my homeowner's with them. I wanted to lower my homeowner's deductible, but the only way they would do that was to bundle my auto insurance. I decided to give them a second chance on the auto insurance, but the final numbers - once I initiated the policy  - came back over 50% higher than what I was originally quoted. That's how they choose to treat a 12-year customer, so I'm taking my money elsewhere.

Yea it sucks they want to peek into that portion of your personal life, but it beats a cavity search from a large man with really cold hands.
