Nothing else but insufficient torque applied on the drain plug. Even if he forgot to put the crush washer on and tightened it to the proper torque it still would not loosen off. he forgot to torque the drain bolt...simple.

There's no way a properly torqued drain bolt could come off on it's own.
Sorry Dennis. The guy who changed your oil last is to blame.
You're very lucky. This could easily have become an Alfredo Mancho incident.

Same thing happened to me a month ago at Don Mills and Lawrence, after a nice trip to the honda dealer on Eglington at the dvp ended up putting a new plug in myself in the middle of Don Mills Mall parking lot and had to use the spare tire jack to get the damn thing off the ground. I was lucky I had a friend driving infront of me, I feel your pain....
