Default Clutch failure? Fork pivot? ACT PP/OEM Disc + PTuning Kit

Was driving on the freeway today and my clutch all of a sudden hits flat. Most awkward situation ever, in a manual car but only left with gas and brake pedal. I pull over, check the pedal. It's on the floor, i prop it back up and its extremely firm. Car will NOT go into gear. Currently awaiting a tow... Question is, what can it be? Am I the only person ever to have their ACT PP/OEM Disc fail? If my disc did fail, can I just toss in another OEM disc and go on my merry way?

Setup:
Clutch: ACT PP + OEM Disc
Mileage: 140k
Miles since boosted: ~30k
Turbo: PTuning Kit, GTX3076R
Power: ~450hp/~300tq on a Dynapack.

Things to check...
Check the Fluid?
Ensure your not leaking
S2000 is a hydraulic clutch systems. Check your fluid level or adjustment bolt might broke or something. Ensure all the parts are working; before going onto the clutch, trans

Check your clutch master cylinder. Maybe there is a leak as mentioned. I had to replace mine a few years ago on my ‘05.

Fluids look good. Dropped off the car to my buddies shop. Will update with what went possibly wrong.

Yup, BNIB OEM everything except the ACT PP.

Can you get into any gear? Could your clutch-fork have possibly slipped off? If you take the rubber boot off on the transmission that covers the slave cylinder hole... check there.

Car will NOT go into gear while its on. While it is on, the clutch pedal feels INCREDIBLY STIFF. But will go into gear when its off.
