On April 10, 1963 a submarine was lost at sea. The submarine was the latest technology and was able to run deeper and more quietly than any other.

I had to look it up... (so no fair posting)
sad story, but apparently was the impetus for the SUBSAFE program that calls for much more stringent safety testing now. At least we learned from THAT mistake.
(we seem to repeating a few other mistakes from the 60's, however)
