Have you had a valve adjustment done? Perhaps it was done badly and this put added stress on the guides. I'm just guessing here.

Lightly bent valves wear out the guides, just before the head of the valve breaks off. Lots of miles, infrequent oil changes, low oil all contribute to guide wear.

Insuficient valve clearance with a botched valve adjustment usually wears the valve seat 1st.

Ever mechanically over rev on a down shift even slightly?

it was once slight over reved by a friend of mine when he shifted from 6-2 at around 75 miles, but i think he caught the clutch before the rpm went all the way up. other than that, there's none i can think of.

Maybe the MPH was a little higher, and he didn't catch it quick enough.

That would be the most likely cause.

If he had completely released the clutch, the engine could have gone as high as just under 12,000 rpm. Even if he did "catch it", he may not have caught it soon enough and you may still have severely over-revved it. You must also consider that this type of scenario would accelerate the engine at a rate far more quickly than the engine would accelerate itself by normal throttle input and this rapid increase in rpm can also be a contributing factor to stressing valvetrain parts, even if the engine didn't get much over redline. This may be a case of a seemingly harmless mistake, coming back to haunt you.

Thanks guys! Most likely that's what caused it. i will call my installer to let him inspect the valves to see if they are fine.
should i replace them even if they look ok? thanks!

If the guides are toast, changing the valves won't fix this. You "might" need a new head. Also, check the spring retainers. They may have been bent or cracked.
