No torque wrench. You'd need one with and access hole through it for the screwdriver and a hollow extension like the tube spanner.
Not true- torque after setting and make sure that it hasn't moved (recheck with a feeler). In my experience, the setting will not change when torquing unless the threads were damaged by previous over-torquing.

If you do it enough times, you can calibrate your hand to the proper torque spec. That's what I've done.

I just made this from a 3 in 3/8 extension and 10 mm socket. Both cost me $0.75 at a yard saile. You can put a torque wrench on there, but I usually torque after and recheck clearance.

I weld them together, but I would guess that you could epoxy them together after sanding surfaces and degreasing. Maybe JB Weld would do the trick.

Because it's off center, your torque wrench will be way off, it's different leverage now.

It is not way off as the difference from center compared to how long the torque wrench is minor. It would also vary depending on what angle the torque wrench is at, so it could be over or under.

For the 3/8 in torque wrench that I have the most it would be off is about 3 %.

0.4 inch off center
15 inch torque wrench

% error = 0.4/15 x 100% = 2.7%

I don't remember the torque for the lifters, but it it's 14 ft-lb then that would be
0.37 ft-lb off worst case depending on angle of torque wrench.

My torque wrench is good for 3% accuracy, so it could be spot on or off a bit more, but not "way off."
