Default Tranny problems in cold and not

Hey everyone, I have an 07 and recently developed a weird tranny issue. I'll describe it as directly as I can. car has about 48,000mi on it.

When cold:
- it's impossible to get into 1st or reverse without some revving up. Even after it's warm it doesn't go in to 1st without some hassle.
- it pretty much grinds into gear if I shift throughout 1-2 2-3 3-4 normally. To remedy this I'll do 1-N, hold for a second, N-2.

When cold and when warm:
- sometimes after I clutch in, 3 and 4 don't want to "let go" of the gear. When normally the shifter will simply fall out of gear if you give it pressure to shift, it holds on, unless I hold the clutch in until it lets go.

When engine is off:
- shifting clutch-in is possible. sometimes a little lockout but generally works.

Attempts at fixing this:
- clutch bleed, power and gravity
- adjusting the clutch pedal rod to both normal and "no-play"

Any ideas? In my couple years of owning the car it hasn't had cold shifting trouble at all. This is probably just a few months old now. Thanks!
