Have you tried running it without the serpentine belt. Just run it for like 15 sec though. Should be plenty of time to see if sound goes away.

Bump for awareness.

I have this same EXACT issue.. been to the dealership multiple times, and ruled out the TCT and valve adjustment. Anyone have a solution to this?

Things done to the car in the last week:

Valve Adjustment
TCT (non-Billman/Ballade)
Oil Change
Ensured oil was topped off
Checked to see if an old oil cap was stuck between engine/trans
Injectors "checked" by dealership (but not cleaned)

The car has been meticulously maintained. No overly hard/aggressive driving - ensure car is warm (20-30 min) before any VTEC (1-2 times per week), no oil starvation or burning in history, and only has 62k miles. 2006 AP2.

Dealership is not making an effort to search for the issue (dropped my car off three times in the last week, not including the above maintenance). Told me to return when and if I see a CEL. However, in my opinion this noise is just too loud to ignore.

These symptoms do not follow the typical injector, TCT, valve, or spark plugs issues addressed a million times in other threads.

Only warm idle, and any throttle makes the noise go away. Cold weather significantly makes the noise louder. The sound is only audible (but very loud) from the cabin. The engine seems normal, is running fine, can redline smoothly, and has no MPG loss.

I actually submitted my car videos to Billman via messenger a couple days ago, and he was unable to diagnose the issue from just video noise, but it seems like other people have had this issue, resolved it, and then never came back to tell us what it was. =(

Did you check the alternator belt and associated pulleys and bearings?

Anything leading you to think that the alternator belt/associated pulleys/bearings might be the culprit?

Please excuse my ignorance, but alternator belt = serpentine/accessory belt?

If so, isn't this noise typically apparent from the engine bay?

I should add that I do have an aftermarket accessory belt (Honda wanted to charge $235 for OEM), but it was replaced less than 3 months ago and is showing no signs of wear. There is also no slack on the belt from what I can tell.

I asked the dealership to check ALL belts and pulleys. They came back with nothing.

If I can't check myself, I at least might be able to steer someone more qualified into what to look for and why it might be the issue.

I already researched piston slap issue.

From what I read and videos I've watched;

It typically occurs on a cold engine.
It can be heard from the engine bay.
It disappears when the engine is completely warmed up.
It can be heard throughout the rev range.

None of these are true for the current symptoms so I mentally just kind of dismissed it. The noise should be going away once the pistons expand enough to fill the gap. The noise I am experiencing persists regardless of the temperature or time the car is running.
