Studder at start up?

Studder at start up?
My car stalls and studders on first startup of the day or after sitting a few hrs. Lights cut out for a second or two. What could it be?
It's a 2005 w/ 52k mi. It started doing it within a few months of purchasing it(about 2 yrs ago) but it was very mild and only when it was cold outside. Lately it's been doing it regularly and it's a very loud grinding noise. Engine still performs as always and I haven't noticed any other differences. I'm guessing it's the spark plugs( I haven't changed them out). Oil is recently changed and within normal levels. It has been sitting over the winter and started up every few weeks(for about 3 months).
