Great thread...figured I'd add to it since I decided to search before posting the same topic again.
Since using the Honda high temp Urea grease shut my TL's slave cylinder pushpin ball up finally so it didn't sound like a 90 year old woman on a rocking chair in my engine bay anymore...
I decided to see if the same would work on the S2000. It was every creakier than the TL and it worked like a charm. Silence, at last. Funny thing is it looks like the slave was changed, but not the ball pin. Where it sits in the fork had the dried dusty rusty looking residue and explains why it was so loud. Undid the 2 12MM bolts...slave comes right out, cleaned it all up, shin etsu on the rubber boot...then fill lube with molykote to preserve my honda grease...
Application on the S2000 is slightly different as the entire ball pin comes out and you need the high temp dark urea honda grease on both ends...just lubed the shaft with the moly (giggle)...hardest part was getting the rubber protective boot back in place but to clean the fork into the tranny and apply the grease, you really need it out of the way. Manually push the pin in to line up the holes and ensure it's sitting in the fork groove...then bolt back up and you're done.
Took my time and took me about 30 minutes including jacking up the car.
I did not grind the nipple down, but the noise is gone just the same.

I have the dreaded creak and stiffness in my clutch and put some fresh grease on the rod/ball end and it didn't change the noise in the slightest. I suspect the noise if coming from the fork pivot or the forked end.
My question is does one have some access to the fork to apply some lube if the slave cylinder is taken out of the way?
You all might get a kick out of the car i have the S2000 engine in - it's a Westfield Mega S2000. It's a really fun car with the wonderful Honda engine and transmission.

x2. I did the procedure outlined here just 2 thousand miles ago and it's getting worse. Pedal feels REALLY gritty when releasing it from full engagement. Starting to wonder if everybody has been missing something with the pedal pivot or the bracket. Haven't had time to pull my seat out and crawl under there yet.

My squeak is persisting as well. The squeak is coming from outside the car, so it is definitely not the pedal. I ground off the bump as recommended, polished the joint and cup to a high shine, re-lubed and reassembled. I am not sure where the squeak is coming from at this point.

Clutch fork? Inner surface of the release bearing?

I had the exact same problem as you guys, I polished the pushrod, greased the cup and it still made the creaking noise. I took it to a well known honda s2000 specialist in my area and it was deemed to be the release bearing. THe pedal feel felt fine when the clutch was depressed, but upon disengagement the pedal was kinda choppy and not smooth. You might have some luck my disengaging the clutch fork and sticking a toothbrush or a pick of some sort and apply some high temp grease to the fork tips. Ever since then the noise has gone away.
