This happened to my AP1, clutch disc and pressure plate were OEM.
The pressure plate had two splines that were bent about a half inch in towards the disc.
Apparently, my release fork had come loose from the retainer/spring and had banged into the pressure plate, bending the fingers in which caught the spring, busting it loose.

OEM discs from Honda say "FCC" NOT Exedy.

THANK YOU. I've changed about 40-50 S2000 clutches now, but somehow people don't want to believe me. I can guarantee that the OP's and S2K.Danny's clutch were the aftermarket Exedy replacement kit. Even the pressure plate is Exedy. (Yes, I know they're stamped Honda, but that's because Exedy rebuilds OEM PP's with new springs and/or friction surfaces) The only way damage like this happens (to either of their cars) is if a foreign object is introduced into the PP, as was with S2K.Danny, or if the clutch has experienced hard launches/drops. This causes excess stress on the spring retainers, they crack/split, and the springs go flying into the pressure plate causing mass destruction. I've seen this exact thing happen to about 5 other S2000's with the same setup and all of them stated their clutch started making noise after they had dumped the clutch. As long as you're not abusing it, these discs should hold up, but when extra undue stress is exerted on the disc things will break.

Well I guess that speaks a bit of volume towards the design of the OEM (made by FCC) friction disc because it seems like the springs would have a muuuuuch harder time trying to free themselves from the FCC friction disc design. The spring retainer design on the Exedy disc looks pretty weak to start with, especially when compared to how the spring is retained in the FCC disc.

I agree. My main complaint about the OEM clutch is the buzz that can occur due to the disc being single sprung (one spring per holder). The Exedy disc is double sprung which I think is why we see those holders cracking/failing more. I think that if Exedy used more of an FCC type holder, we wouldn't see so many failures.
