when my motor failed I had chunks of bearing material in the oil pan but none drained out, if they chunk they get trapped in the oil pan and don't necessarily drain out as they are heavy, kind of like panning for gold. Not saying that is your situation but it can happen.

Default I have a brand new tensioner installed

I'm not sure about the chain guide. I suppose that could come loose.
