This should not have happened. That's what the clutch/starter interlock switch is for - to require you to have the clutch pushed to the floor to complete the circuit for the starter. If you were able to operate the starter with the clutch up, then that interlock switch is either faulty or it is not adjusted correctly.

How about a short "grinding" sound immediately after the engine engages? (after pushing the start button when the car was NOT already running) This has happened a few times recently, but not consistently.

I'm taking her in for a service this week and want to have it looked at. Would this be the flywheel?

Although not a cause for alarm, this "might" be an indication that either you are holding the button just a tad too long OR, the bendix is not retracting cleanly after you release the button. This would mean that the flywheel is now driving the bendix gear rather than the other way around. There could be several reasons why this would happen, but try to notice if maybe you are cranking just a second or two too long.
