Car was bought used with 9800 miles. I've put about 4000 more on. No aftermarket rims were ever put on. I honestly think I'm the first one to take off the wheels; It has the original SO-2's on it. I'm the third owner and the two guys before me were in there forties (I'm 25); weekend cars for them.
Definitely only a star pattern.

Swap the wheel to the other side and see if the problem follows (I know the tread pattern will be wrong way round, but ignore that for a short test).

Even though spec calls for 80 ft-lbs, seems like a lot of people torque them between 80 - 90

(search for "lug nut" in the page, its about 1/2 way down)
I myself torque them to just a bit above 90 ft-lbs and have never had a problem with them coming loose (OEM ap1 wheel and lugs).
edit: btw, can't stop staring @ your avatar.

Let this be a lesson, even to those who've been in the game for a while. I've been working on cars for roughly 8-9 years and I pulled the ultimate noob mistake.
Check you torque wrenches. Make sure they say FT-LBS NOT IN-LBS.
I borrowed a torque wrench as mine is getting serviced. I assumed it was in FT-LBS and didn't even think twice to check it. Thank God that only a couple of lugs came loose and nothing worse. Take care all and happy tuning. Thanks for all of the suggetions and quick responces everyone.
