So it happened to me today. I swear I had it on tight too, we'll see what the rubber o-ring looks like if I manage to fish it out!
I thought I had lost it forever but after reading this thread I had another look and here is where it's currently sitting.
It is on the UK passenger side of the rocker cover down below against the firewall I think. However it was dark so I could only see it just enough with a torch to realise I wasn't going to be able to grab it with my hands!
Looks a bit of a tricker location than the other photos I've seen!! I never thought this would happen to me! FML!

Man I'm glad I found this thread. I've had this annoying rattle since I bought the car. Now that I'm running stock exhaust and airbox the rattle was unbearable and I was convinced something was very wrong. Weird thing was that I only heard the noise in the cabin, but not when popping the hood and listening to the engine. My cap was not visible at all when looking down the firewall. I found an old handbrake cable in the garage, put the car on jacks and started prodding around above the gearbox. After a while I heard a plastic noise when pushing the cable. I made a loop with the cable and was able to push the cap forward so it became visible at the firewall. I got down under the car holding the cap in place while my girlfriend fished it up from above. About three hours of swearing, pondering and losing arm skin So satisfying to finally be rid of the noise.

Ha ! ^^^ looks little my oil cap I jacked it up tonight and retrieved it once i spotted it..
It turned out that my recent engine had no gasket in the cap,I thought it dropped on the road..
The noise was **** annoying and sounded like my clutch installation was the problem.
