looking at majestic honda, theres flywheel, friction disk, pressure disk, bearing clutch pilot?? is that the throwoutbearing?? what else do i need?? can i still get my flywheel resurfaced??

Depending on the damage on the flywheel you can get it resurfaced. You will want new throwout bearing(pilot bearing), clutch dish and pressure plate as well.
would i need to buy the bolt and washers thats listed on there also? or will the necessary stuff come with the clutch?

i wouldn't say it's totally gone yet. my clutch was glazed from tracking and behaved exactly like yours does on any fast shift. i was getting sort of annoying so i changed it out, but it looks like i had about 50% life left or more on the disk. it would have been fine for daily driving for quite a bit longer. i had 60k miles on it too.
unless you're slipping in the 4th/5th gear test, i wouldn't worry.

I don't think this is because of your clutch wearing out. There is a very brief delay between releasing your clutch pedal and your clutch fully engaging. If you get on the throttle too quick before it engages completely then it will slip. The delay is longer in AP2's than in AP1's, but they both do it if you get back on the gas as you're letting the clutch out. I'll bet if you make sure the clutch pedal is completely released before getting back on the throttle this will not happen.
