Tinker's explanation seemed pretty definitive.

I'll be under the hood on the weekend so I might try unchecking the "All On" option and see how well it starts.

BLKMGK knows his stuff, but he also likes to be a bit elusive. I assume there is some setting for the injectors or ignition that stops them squirting/firing under a given crank RPM. Perhaps if this setting is raised it won't squirt/ignite until the crank is turning. I'll have a gander this weekend.
