Just got a call back from invidia, they said that the bung is in the wrong place, and they have had a few incidents with that. They're sending me a new one soon.

I had the same issue w/ my Invidia test pipe. I found out when I took it to my muffler shop for install. What I had the welder do is cut the pipe 2 times, right on the edges of the section that has the hole for the o2 sensor, rotate it to make the hole horizontal when installed and then welded the piece back to the pipe.
