When they work loose, the fuel / air mix escapes and ignites up the spark plug well, melting the plug and the coil pack.

Check the threads of the plug to make sure you didn't damage the cylinder head.
Funny you say that, my car was at a local performance shop getting some work done.The coilpacks had to come off in order to perform the work I asked and paid for. When I took the spark plug cover off all four coilpacks were not tight. (Coilpack screw was not tight). This was a reputable shop but I guess everyone makes mistakes. The plug seems to be tilted towards one side not allowing me to get a spark plug socket to remove it, I guess off with his head now.
