Default Couple of issues I need help with!

My new-to-me S has been great to me the last few weeks but I've run in to a few minor issues. I've done multiple searches(which have been most helpful for other issues!) but have yielded no results specific to my issues. I will list them numerically.

1. I get the P0411 CEL; I removed the factory airbox a couple nights ago to replace the serpentine belt. I am fairly sure I reconnected the vac hoses correctly. When I start the car, I hear the air control valve(I think) turning on and checking, and when it turns off, that's when the CEL comes on.
-SOLVED: One of the vac lines connected in the upper right corner near the fuel rail in this diagram was disconnected:

I can't figure out how I disconnected that, but I did. Another lesson of tracing your lines according to the diagrams, folks.

2. I keep getting the P1456 CEL every 4-5 days; the gas cap is on accordingly but I still get the error. There are no fuel leaks anywhere, however, when I fill up with a fresh tank, the gasoline smell is VERY strong for several days. I checked under the gas cap and there is no nozzle hole or anything; there's just a straight hole in to the gas tank itself. Is that how the s2000s are supposed to be?

3a. I hear a rattle from underneath the car. I suspect it may be a lost oil cap because the current oil cap I have specifies 5w-30 for the car, but I think it's supposed to be 10w-30. I'm thinking the previous owner dropped it and simply placed a different one on. Where would one look for dropped items in the engine bay or under the car?
3b. The rattle may be something else, though, because it sounds more like a loose heatshield rattle.

2. You may need to buy a new cap. Or you can try giving it an extra turn each fill-up.

3. They get jammed between the firewall and the tranny.

Solved #1 thanks to the thread negcamber provided. I used the diagram to trace the lines.

Drove around a bit today; the fuel smell is very intense when I get down next to the rear driver side wheel. Like, it's coming from within the wheel well or something. However, there is no leak.
