clutch issue?

clutch issue?
so ill start of with my car info
00 ap1
77,xxx miles
changed tranny fluid about 20,xxx miles ago
my problem:
over the last couple of days my clutch seems like it has started to take a dump but idk, while driving its harder to get it into gears while shifting sometimes grinding the gear before it will go in, and when i come to a rolling stop if i leave it in first with the clutch all the way in when i come to the stop it jerks/clunks as if the clutch isnt disengaging completely. now these problems have just started to happen over the last couple of days but i have noticed the clutch not grabbing as hard etc. another thing i noticed today was the clutch pedal was softer and had more play in it then what i remember it being say last month. does this sound like my clutch finally going out or another issue like a simple clutch bleeding or fluid change, any info will help so i can start to trouble shoot thanks.
