How is your clutch freeplay? an over adjusted rod will give the results you are experiencing.
At standstill, make sue the master piston is not pushed in at all. you will feel the small built in freeplay of the master with one finger pushing the clutch pedal. check it.
Pedal must feel firm, right from the top after you pass the freeplay point. if not, it leads me to believe you have air.

I had the same symptoms happen to me - rough to shift, even more difficult to shift, then nothing. Clutch pedal basically made no difference when pressed.
It was the shift fork retaining clip/spring. The shift fork bounced around a little inside, & broke one of the springs off of the clutch disc. Glad you got it figured out.
