I've suffered a severe 2nd gear grind that resulted in my original transmission being rebuilt and then ultimately replaced altogether.
I've experienced a much milder 1-2 upshift grind in the new transmission (4000 miles on it with only two mild to slightly moderate grinds). The transmission was extremely notchy with the Honda MTF so I switched to Redline and it has solved most of the problems. As Xviper mentioned, it seems to do it mainly when the transmission is hot. Additionally, as Xviper suggested, I also changed the fluid in the clutch master cylinder (completely bled it) and that seemed to improve the clutch pedal response. The 1-2 shift takes some time to get right and requires a little more discipline and good technique.
