Shoulder seat belts slide off
I'm only 5' 8" but my shoulder belt routinely slips off the top of my shoulder to the side of my arm. Obviously this makes me a little nervous from a safety standpoint. Does anyone else have this problem? Wouldn't all you tall drivers have even more trouble than me? I wonder if the seat's shoulder belt bracket can be moved up an inch or so by the dealer?

Well I'm also 6'1" and the belt does not fit me well. I'll often leave it at the side a bit just for improved comfort. When I was driving the car all day long, every day, last year I noticed that the belt was digging into my shoulder after a while. It's just one of several minor annoyances I gladly put up with.
As far as safety goes, it's not a problem unless you crash your car.

A month and a half later, the belt sliding off my shoulder is still annoying and very distracting when I drive. Could someone please recommend a good seat belt cover/pad? I want something that is not for cushioning against my chest (i.e. just for comfort). I need it to keep the seat belt tight against my chest, over my shoulder, and keep it from slipping off the shoulder toward the outside of my arm.
Alternatively, I need a new seat belt guide that can be mounted higher on the upper seat back (or can I move the stock guide?). Thanks everyone.

As an alternative, a "locking clip" that secures the lap portion of the seat belt may change the angle enough so that a new guide or guide position is not necessary. The "locking clip" fits over both the shoulder and lap belts immediately adjacent to the buckle insert. It is used to securely fasten infant/kiddie seats in rear seats which have a shoulder/lap belts that tighten only when a body is moved forward quickly. I installed it because I wanted to feel positive restraint at all times. However, only the lap portion is secured.
The "Locking Clip" is available at infant stores, made by evenflow and costs about $3.98. Might be worth a try.

I'm guessing that the farther forward the seat is, the more likely the seat belt is to fall off your shoulder.
Maybe some blocks on the pedals would help.
Try leaning the seat back slightly. If that doesn't work trying raising the guide on the seat. I haven't heard of anyone doing this, but it can't be that difficult.
