I don't know what other people will tell you but according to the VTEC Challenge BM video, the president of Spoon believes that tranny oil should be changed at the same intervals as motor oil. Logically speaking he says that your tranny takes just as much a beating on a daily basis as your engine. Changing it out allows you to monitor for damage to your tranny as well. It's really subjective, I believe Honda recommends somewhere around 12k miles, but am not positive on that. I've got about 15k on my car and have changed it about 4 times in that time span. It's a relatively simple thing to do, a lot of us have switched to GM synchromesh FM, it'll cost you about $12 a bottle and I think you need about 2 bottles, if my memory is correct.

Tough one. 12k I'd say is MAX. Between 6k and 8k is more like it. Problem with tranny oil is when it comes out, it's usually clean. But don't be fooled, there are anti-shear additives in all gear oils (microscopic pieces of plastic) that wear out, which can't be seen.
