So I'm assuming you don't have the title in hand? How would this work?

 

You get a loan or give him cash, he pays off his loan, he gives you the title. Thats it. Your bank will understand that he has a loan on it and they will not receive the title immediately. You will have a bill of sale showing he sold it to you until you get it. Would be best to have it notarized.
