You can find numerous reports where Toyota / Lexus struggled with this problem, settling a number of claims. Apparently cooling problems led to sludge build even when oil was changed as recommended. It has been reported that engine changes have been made since then to rectify the problem. I can see that a minor shortcoming can be made worse by some driving habits (excessive idling, etc.) so a manufacturer can be faced with a suprising number of failures they did not expect.

I have a 3.3L V6 in my sienna, I was worried b/c I use the longer change interval 6K or 7.5K. But I keep an eye on my oil even at 6K it looks "new" so I hope I'm alright.
Problem with every 3K is that'd be 5-6 times a year just for the Sienna so I try to put it off as long as possible. I have 4 cars so if I changed them all at 3K it'd be at 13 oil changes/year.
