Try getting a new or other used MAF. I've had a similar problem with my old 240 where it would stall out of nowhere and had a constant cutoff on the fwy. I cleaned the old MAF like 1000xx times and thought it wasnt the issue but replacing it with a different MAF fixed all the staling, sputtering, cutoff issue.

When your starter doesn't engage it will just "spin". It will sound like a fast winding.

altho this doesn't explain the low IAC voltage, it might be a good idea to clean out your fuel system anyway

I know little about cars, but I'm also thinking this has something to do with your fuel system.

I had a Mitsubishi Eclipse and had the exact same symptoms as your car, turns out I lost the fuel cap when I was filling up and was losing compression in my fuel tank or something like that.I forgot already...

i've listened for the fuel pump to prime before cranking, but haven't noticed any odd noises or whine.

i'm going to try and test the fuel filter and the fuel pump next.

thanks for the help so far guys.

One dead injector won't cause it to die and start. It will idle rough.

Start by swapping your iac.

Next time your car does this, quickly check for fuel pressure and spark. This will be your first step.

i tried swapping the IAC with a lower mileage one. the swap resulted in the car stalling out immediately every time. it would start up on the first time though.

Did you check the ecu after you swapped the IAC?

Have you checked timing?
