I have used The Original Underwhere Company for t-shirts several times over the past 5 years. They're located in Redwood City and they do an excellent job at reasonable prices.
I usually pay about $10 per shirt for single-color, single-side artwork in quantities of 25-30. In higher quantities, the cost of multiple colors or halftones end up being about the same, i.e. $10-15 per shirt.

My friend owns a heat press specifically made to press iron-on decals to t-shirts, so we can print out decals from somewhere like kinko's and iron them onto the front/backs of the shirts. The sleeves might be a littler harder
