What caused the scoring in #4? Is that the only cylinder damaged?

It's not scoring actually, a valve broke and hit the sleeve, so there is an indentation in the side of the sleeve.

My bad then. Right next to it, it appears as though there is a thin vertical line suggesting scoring.
